Each near-miss entity investigated through the system is defined through a specific case definition agreed at national level. Specific pregnancy complications are used as the starting points and then for each complication, a set of markers (clinical conditions, laboratory values, or interventions) of life-threatening complications are defined. The number of near-miss cases on a monthly basis is identified through routine electronic reporting sent by officially nominated reporting clinical staff (reporters) in each participating maternity unit. The reporting is based on the standardized case definitions. The number of cases meeting the case definition each month is reported. Reporters are asked to indicate whether there have been any women with specific severe morbidities admitted to their unit over the previous month. They are also asked to fill in the reporting page indicating if there have been no cases. When a participating maternity unit reports a near-miss case, it then fills in a Case Data Collection Form (DCF) through the same confidential OSRS App electronic system. The information reported in a standardized Data Collection Form is anonymous, with no names attached, and include the list of data items/variables agreed at national level.

Also noteworthy is their supportive and helpful collaboration, their flexibility and responsiveness throughout the project, and their strong focus on continuous improvement rather than a one-off delivery.

Of course, in a project of this scale, there were also challenges and not always smooth phases—which is normal given the complexity. However, these were always handled in a constructive and professional way, and appropriate solutions were found together.

Overall, the collaboration with REEA was very positive. If anything, faster alignment in some complex phases and more proactive communication around potential delays could have further improved efficiency.

However, whenever such situations arose, they were handled well and resolved constructively. In general, the cooperation was strong and solution-oriented throughout the project.

We are currently engaged with REEA on a significant refactoring effort to scale the platform for continued growth, which reflects our confidence in their ability to evolve with our needs. Perhaps the most telling measure of success is the longevity of the relationship itself. I have worked personally with REEA for approximately 18 years across multiple organizations, since 2018. That level of continuity and trust is not something you maintain without consistent quality and commitment.
