The project began with a discovery phase where the team worked closely with Mediktor to understand the patient journey, care delivery process, and operational challenges before designing the solution rather than simply configuring the platform out of the box. From there, CloudCache implemented several core Health Cloud capabilities, including Patient Cards, Care Plans, Care Teams, Clinical Timeline, Provider Management, Households & Relationships, and the Health Cloud Console. They also built custom patient intake workflows, referral management processes, appointment tracking, and automated follow-up processes designed to reduce manual work.

A key deliverable was the 360-degree patient view, giving care coordinators a single screen to access patient demographics, medical history, care plans, referrals, appointments, and interactions, which made coordinating care across multiple teams significantly easier. CloudCache also implemented workflow automation using Salesforce Flow for patient onboarding, referral assignments, care plan updates, follow-up reminders, and case routing, along with reporting dashboards providing real-time visibility into patient outcomes, care coordinator workload, referral status, and operational KPIs. The project was delivered on schedule between January and May 2025, with CloudCache's consultants proactively recommending best practices throughout rather than simply implementing initial ideas as given.

What were the measurable outcomes from the project that demonstrate progress or success?

Since go-live, Mediktor has experienced significantly better patient data visibility, with care coordinators now able to access a complete patient picture from a single screen rather than piecing information together across systems. Collaboration between care teams has improved, patient onboarding has become faster, and referral management has been streamlined through the new automated workflows. User adoption across the organization has also been strong, reflecting how well the solution fits our actual care coordination processes.

Prior to the engagement, these systems operated independently, which meant manual data entry and reconciliation across our sales, operations, and warehouse teams. The goal was to build automated data flows between these platforms that would eliminate this manual work and improve accuracy across the full order-to-fulfillment process.

The engagement began with a discovery phase, where CloudCache worked closely with our teams to understand our existing business processes before designing any technical solution, this ensured the integration architecture matched how we actually operate rather than following a generic template. From there, they designed and built out the MuleSoft integration layer connecting Salesforce, SAP, and our warehouse system. 

The key deliverables included automated order processing, so customer orders placed in Salesforce flowed directly into SAP ERP without manual re-entry; inventory synchronization, keeping stock levels consistent and accurate across our sales and warehouse functions; and shipment tracking integration, giving our teams real-time visibility into order fulfillment status across systems. CloudCache tested and deployed these integrations within the agreed project timeline, and remained responsive throughout for questions, troubleshooting, and change requests. The one area for improvement was around knowledge transfer documentation at project close, though the team remained available to answer questions as needed.

Since implementing the integrations, Biesse Group has seen a meaningful reduction in manual data entry across our sales, ERP, and warehouse teams. Data accuracy has improved significantly, as information no longer needs to be re-entered or reconciled by hand across separate systems. Our teams also spend considerably less time switching between platforms to track order status, since order, inventory, and shipment data now flows automatically between Salesforce, SAP, and our warehouse management system. Overall, this has made our daily operations more efficient and given us greater confidence in the accuracy of our business data.
